Individuals, state committee members, inspectors, and Tree Farmers helped design the new form. A full review of the revised 004 Form will be available in the Inspector Training Refresher Course and during the regular Inspector Training Workshops provided by State Tree Farm committees.
New modifications:
- The 004 form now consists of 2 parts
- Part one: a carbon copy form for collecting information on the Tree Farmer, the Tree Farm, the pre-printed identification number, the inspector, the management activity, and the management recommendations.
- Part two: an easy, fill-in-the-blank, yes or no Auditor Verification attachment. Now a requirement of the inspection process, this attachment outlines the AFF Standards of Sustainability for Forest Certification (AFF Standard).
The 2004 Reinspection Forms will be printed on the new 004 Form. Additional details are available during upcoming trainings.
